American Kidney Fund - Medicare Part D Program for Prescription Bone Medications

• For dialysis patients enrolled in a stand-alone Medicare PDP or MA-PDP without alternative drug coverage.
• Covers prescription bone medications for patients on dialysis.
• Formulary includes Renvela^® (sevelamer carbonate) and Hectorol^® (doxercalciferol capsules).
• Patients can receive up to a $2,000 grant per calendar year.
• Limited Income Subsidy (LIS) patients can receive up to a $500 grant per calendar year.
• Grant counts towards True Out of Pocket (TrOOP).

Healthwell Foundation

• For eligible patients with the diagnosis of “Secondary Hyperparathyroidism” in addition to other disease states.

Hectorol Start Card

• The Hectorol Start Card works like a coupon and is good for up to  80% off out-of-pocket costs for privately-insured and cash paying patients.
• Hectorol Capsules only
• The Hectorol Start Card is not available for patients with government funded drug coverage (Medicare, Medicaid, TriCare, etc.).
• Each card is good for up to three prescriptions and does not expire until December 31^st 2009.
