Insurance Verification Services

Renassist can contact your patients’ health insurers and prescription drug plans to determine available coverage for Genzyme’s renal products.  In addition, the Renassist team can help facilitate prior authorizations and exceptions.

How to submit a request for verification

• Your patients must first provide consent to the release of their personal health information.  This can be done by having them fill out one of the following: Your office or company standard release of health information form, the Genzyme's Patient Authorization for Release of Information to Renassist Consent or Renassist Insurance Verification Form.
• Gather the patients’ insurance and demographic information to complete Renassist Insurance Verification Form (If using another patient consent authorization you can also submit your own computer generated information report.)
• Fax or email the patients' verification request information to Renassist at 877-363-6732 or email PAP@genzyme.com.

Renassist will contact the following:

• Commercial Plans
• State Medicaid
• Medicare Part D Plans
• SPAPs (State Pharmacy Assistance Programs)
• Kidney Funds
• Government plans, i.e. VA, TriCare, FEP, etc.

Renassist will help to identify information on the following:

• Co-pay ($) or Co-insurance (%)
• Deductibles
• Mail Order
• Out of Pocket Maximums
• Pharmacy Caps
• Information on Pill Limits
• Medicare Part D Coverage Gap
• Limited Income Subsidy (LIS) Enrollment
  

Renassist will also facilitate the processing of:

• Prior Authorizations
• Formulary Exceptions
• Tier Exceptions
• Appeals
